Catecholamines

Catecholamines (CA) are a class of neural substances containing catechol and an amino group. Catechol and the amino group are combined through enzymatic steps from L-tyrosine in sympathetic nerves, adrenal medulla, and chromaffin cells. The core value of its clinical detection lies in assisting the diagnosis of neuroendocrine tumors and evaluating the stress state.

Core Applications and Clinical Purposes

1、Assist in the Diagnosis of Pheochromocytomas and Paragangliomas (PPGLs) ---- This is the most core and important clinical application.  

PPGLs are tumors originating from the adrenal medulla (pheochromocytomas) or extra-adrenal sympathetic ganglia (paragangliomas), which secrete large amounts of catecholamines intermittently or continuously, leading to typical symptoms such as paroxysmal or persistent hypertension, headache, palpitations, and profuse sweating. TDM provides key biochemical diagnostic evidence for PPGLs by measuring the concentrations of catecholamines and their metabolites in blood or urine. 

2、Evaluate Sympathetic Nervous System Activity  

In severe stress states such as major trauma, shock, heart failure, and hypoglycemia, the body's sympathetic nervous system is excited, and the adrenal medulla secretes large amounts of catecholamines. Monitoring their concentrations can be used to assess the intensity of the stress response and the patient's pathophysiological state. 

3、Identify the Causes of Hypertension  

For patients with refractory hypertension or hypertension accompanied by paroxysmal symptoms, measuring catecholamine levels helps rule out PPGLs as a cause of secondary hypertension.
